📋 Description

• Provide In-Home Health Evaluations in members’ homes • Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals, and/or flights • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Visit people nationwide and connect them to appropriate care • Make referrals to health plans and encourage involvement of members’ loved ones • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to safely perform duties in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants • Ability to conduct virtual visits as needed
