📋 Description

• Provide in-home health evaluations in members’ residences • Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights • Conduct approximately 45-minute visits with one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services • Build trusted relationships and help improve health outcomes • Make referrals to members’ health plans and encourage involvement of loved ones when appropriate • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, airborne allergens or irritants • Willingness and ability to travel locally and within licensed states, up to 100% of the time • Ability to conduct virtual visits as needed
