📋 Description

• Provide full-time In-Home Health Evaluations in members’ homes • Travel up to 100% of the time across a defined coverage area • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits with one-on-one health conversations • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Provide Signify Health services, including Diagnostic & Preventive Services products • Conduct virtual visits as needed • Make referrals to health plans and encourage participation of members’ loved ones when appropriate • Help ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Primary residency in the state of Virginia • Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within Virginia and flex to other areas (states) of business need • Able to travel locally and within licensed states, up to 100% of the time • May require overnight hotel stays, car rentals and/or flights • Comfortable and able to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • MD or DO qualification explicitly indicated by the required and preferred qualifications heading • Ability to conduct virtual visits as needed
