📋 Description

• Provide in-home health evaluations in members’ residences • Travel primarily, up to 100% of the time, across a defined coverage area • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medication and medical history • Perform physical evaluations • Provide ordered diagnostic tests, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Build trusted relationships and help bridge gaps in care • Travel locally and within licensed states, including possible overnight hotel stays, car rentals, and flights

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within California and flex to other areas of business need • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, up to 100% of the time • Ability to conduct visits in homes that may contain household pets, secondhand smoke, airborne allergens, or irritants
