📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute one-on-one visits • Review medications and medical histories • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting • Filing of the appropriate attestation with the state of Minnesota • Board certification • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants
