📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute one-on-one visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Provide education and answer members’ health questions • Visit people nationwide to connect them with appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services products • Conduct virtual visits as needed • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Able to cover Yakima County and surrounding area • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other airborne allergens or irritants • Able to travel locally and within licensed states as needed • Able to conduct virtual visits as needed
