📋 Description

• Provide approximately 45-minute In-Home Health Evaluations • Engage with health plan members in their homes and build trusted relationships • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Help identify connections to appropriate care and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) Snohomish • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
