📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with health plan members in their homes and build trusted relationships • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Help identify connections to appropriate care and bridge gaps in care • Travel locally and within licensed states as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Able to cover Bremerton MSA and Kitsap County • Able to travel locally and within licensed states as needed • Able to work safely and comfortably in homes that may contain household pets, secondhand smoke, airborne allergens, or irritants • Nurse Practitioner or Physician Assistant qualification
