📋 Description

• Provide patient-centered oncology care • Conduct motivational interviewing and health coaching • Manage ultra-complex cases and provide consultations • Oversee RN-managed complex cases • Handle advanced clinical escalations • Prepare and review appeals and requests • Coordinate care with providers, specialists, testing facilities, and agencies • Assist with phone triage and outreach • Educate members and caregivers about diagnoses, treatment plans, symptom monitoring, medications, survivorship, and supportive services • Conduct comprehensive telephonic oncology assessments • Develop individualized care plans • Provide evidence-based clinical consultation and symptom-management support • Monitor treatment adherence, side effects, changes in condition, and emerging clinical concerns • Escalate urgent needs appropriately • Identify gaps in care and contribute to quality, population health, and program-improvement initiatives • Maintain accurate, timely documentation according to privacy, regulatory, accreditation, and organizational standards • Collaborate with the Oncology Care Model team, treating providers, and interdisciplinary care teams to improve outcomes and member experience

🎯 Requirements

• 3 years of oncology experience • Active, unrestricted Registered Nurse license in the state of residence • Active, unrestricted Nurse Practitioner license in the state of residence • Ability to obtain and maintain additional state licenses as required by the program • Clinical knowledge of oncology treatments, treatment-related toxicities, symptom management, and care coordination • Excellent telephonic communication, clinical judgment, documentation, and member-engagement skills • Ability to work independently in a remote environment • Ability to navigate multiple clinical and technology platforms • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program
