📋 Description

• Deliver holistic, evidence-based remote patient care through a virtual technology platform • Assess, diagnose, treat, and manage health problems for patients aged 18 months and above • Provide health counseling, education, treatment, and disposition planning • Provide pregnancy prevention, STI prevention, safer sex, contraceptive care counseling, and medication management • Evaluate primary care, acute, chronically ill, and transitional care patients • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in an EHR according to MinuteClinic policies and procedures • Coordinate care with internal and external colleagues, including the broader patient-centered medical home • Collaborate with Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice • Work 40 hours per week on a day-shift schedule including every other Saturday and Sunday, from 12:00 p.m. to 8:30 p.m.

🎯 Requirements

• Current, unrestricted license in one of: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV • Minimum of two years of medically-relevant experience or equivalent • Completion of a Master’s Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem solving ability, adaptability and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Broadband connectivity, a quiet setting with a neutral background, and ability to uphold patient privacy for remote care delivery • Ability to interact with customers remotely, use computers and phones, speak intelligibly, and hear patient responses • Ability to view and read computer screens and other electronic devices
