📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical history • Educate members about their health and answer questions • Help close gaps in care • Provide connections to the right care for members’ unique needs • Consistently engage with regular manager, team, and clinical meetings • Be accountable to Signify Health operational, clinical, and member experience KPIs and metrics • Deliver Video Visit telehealth services only • Interact with health plan members remotely • Operate computers and phones while conducting visits

🎯 Requirements

• Full-time, board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • FNP or AGNP certification preferred for nurse practitioners • Maintain board certification • Able to obtain and maintain multi-state licenses in coverage area, as required • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Provide 40 hours of availability weekly • Consistently provide 320+ capacity units per month • Consistently achieve 220+ completed visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled • Conduct visits from a quiet setting with a neutral background • Uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Ability to view and read a computer screen, iPad, and other electronic devices
