📋 Description

• Review and triage provider requests and member cases • Conduct member outreach to confirm delivery of expected home care services and adequate member support • Escalate identified concerns to clinicians for follow-up and intervention • Communicate with healthcare providers, members, and internal teams to support quality care outcomes • Research and resolve case-related issues while ensuring compliance with regulatory and company standards • Maintain accurate documentation • Provide administrative and customer support for operations • Support members’ transition from acute/post-acute care to the home setting alongside nurses, medical directors, providers, and care teams

🎯 Requirements

• Knowledge of basic medical terminology and healthcare concepts • Strong customer service and communication skills, including telephone support • Call Center experience is required • Excellent organizational and problem-solving abilities • Proficiency with Microsoft Office applications, including Word and Excel • Ability to work effectively in a collaborative, multidisciplinary environment • High School Diploma required • Ability to work Monday–Friday, 8:30 am to 5:00 pm Eastern Time Zone
