• Conduct monthly outreach calls to assigned members to monitor well-being and document updates • Assist Care Managers in completing and submitting service authorization requests • Follow up with Primary Care Provider (PCP) offices to obtain required documentation for service requests • Coordinate with service providers to confirm delivery and implementation of authorized services • Support members with Medicaid recertification processes to ensure uninterrupted enrollment and benefits • Assist in scheduling and coordinating home visit assessments as needed • Compile and share relevant member information and reports with other departments to support interdisciplinary collaboration • Maintain accurate and timely documentation in care management systems
• 2–3 years of experience in healthcare coordination, case management support, or administrative roles in a clinical or social services setting • Bilingual- Mandarin and/or Cantonese - May be required to take and pass a language assessment - verbal and written • Strong communication and interpersonal skills, especially with vulnerable populations • Ability to manage multiple tasks and follow through on assignments independently • Familiarity with Medicaid processes and service authorization workflows • Proficiency in using electronic health records (EHR) and care management platforms • Detail-oriented with strong organizational and documentation skills • Ability to work collaboratively with clinical and administrative teams • Computer literacy to navigate through Internal/external computer systems, including Excel and Microsoft Word
