📋 Description

• Assess, plan, implement, and coordinate case management activities with members • Evaluate members’ medical needs and facilitate overall wellness • Develop proactive courses of action to address issues and enhance short- and long-term outcomes • Use clinical tools and information/data review to evaluate members’ needs and benefits • Apply clinical judgment to reduce risk factors and barriers and address complex health and social indicators • Conduct assessments using information from various sources, including co-morbidities and multiple diagnoses • Holistically assess the need for referrals to clinical resources and interdisciplinary team members • Collaborate with supervisors and healthcare stakeholders to overcome barriers and meet goals and objectives • Follow case management processes in compliance with regulatory and company policies • Use motivational interviewing to maximize member engagement and determine health status and needs • Work on behalf of the client and the member’s provider

🎯 Requirements

• Registered Nurse residing in the Continental United States • Unrestricted nursing license with multi-state/compact privileges • Ability to be licensed in all non-compact states • 3+ years of clinical/nursing practice experience • 1+ year of medical case management, care coordination, or discharge planning experience • 3+ years of experience with personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications • High-speed internet with ability to hardwire the computer • Direct/hardwired internet connection to modem/router within 7 feet of computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines • Ability to work within the state and city where currently residing • Ability to use multiple monitors and navigate multiple applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace tools • Ability to access secure systems such as VPN and EHR portals • Ability to follow computer security practices, manage strong passwords, and recognize suspicious emails or links • Ability to troubleshoot common technical issues independently • Associate's degree in nursing required • Telephonic case management experience preferred • Excellent communication and interpersonal skills preferred
