📋 Description

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews • Develop individualized care plans addressing behavioral and physical health needs • Coordinate care with providers, community organizations, and support services • Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes • Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up • Utilize Motivational Interviewing techniques to engage members and promote sustainable behavior change • Coordinate psychosocial wraparound services and connect members with community resources • Support cost-effective care through proactive care planning and resource utilization

🎯 Requirements

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field • Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent) • 3+ years of post-master's direct clinical experience in a hospital, ambulatory, outpatient, or similar clinical setting • 3+ years of experience treating mental health and substance use disorders • 3+ years of experience utilizing Motivational Interviewing techniques • Case Management experience • Ability to simultaneously talk on the phone and document in clinical systems • Strong technical proficiency and comfort working in a virtual environment • Crisis intervention experience • Managed Care experience • Utilization Review or Utilization Management experience • Discharge Planning experience • Experience working with complex behavioral health populations
