📋 Description

• Complete In-home Evaluations, Focused Visits, and Video Visits. • Meet Signify Health operational, clinical, and member experience KPIs. • Test new Diagnostic & Preventive Services and clinical workflow changes and provide feedback. • Provide 40 hours of availability weekly, 128+ capacity units per month, and reserve one assigned day per week for coaching and mentorship. • Maintain an 88+ completed-visit-per-month pace. • Guide new-hire clinicians during their first 90 days. • Facilitate virtual and in-person shadowing opportunities. • Provide best-practice guidance on clinical workflows and diagnostic and preventive services. • Support active clinicians through webinars, office hours, and real-time in-field assistance. • Respond to non-urgent clinical inquiries and escalate issues to Clinical Leaders or Operations Managers. • Serve as an early adopter for new services and workflow changes. • Meet regularly with Clinical Leaders and participate in manager, team, and clinical meetings. • Provide mentoring and coaching during assigned rotating evenings and weekends. • Maintain required training, affiliations, compliance, supplies, and diagnostic equipment. • Travel in-region and locally as business needs dictate. • Accommodate field observations and ride-along experiences.

🎯 Requirements

• Clinical background and state licensure, such as MD/DO or NP/PA, required. • Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies. • Ability to inspire, motivate, and mentor peers. • Ability to share leading practices and first-hand experience. • Understanding of managing inquiries and escalating performance issues to leadership. • Understanding of upholding high-quality member care across teams. • Experience completing In-home Evaluations according to operational, clinical, and member experience KPIs. • Experience mentoring teams and providing 1:1 professional and personal support. • Experience with Signify Health IHE tools and processes. • Experience developing and presenting group presentations. • Multi-state licensure preferred. • 5–7 years of clinical experience, preferably in primary care, chronic disease management, or a related field. • Minimum 6 months performing Signify Health in-home health evaluations preferred. • Availability for 40 hours weekly and assigned rotating evenings and weekends. • Ability to travel locally and in-region as required.
