📋 Description

• Work intensely as a telephonic case manager with patients and their care teams for fully and/or self-insured clients • Apply and interpret applicable criteria, clinical guidelines, standardized care management plans, policies, procedures, and regulatory standards • Assess benefits and member needs to ensure appropriate administration of benefits • Apply clinical judgment and strategies to reduce risk factors and barriers and address complex health and social indicators • Assess co-morbid and multiple diagnoses affecting functionality • Use motivational interviewing to maximize member engagement and determine health status and needs • Identify and appropriately escalate member needs according to guidelines and protocols • Actively reach out to members to collaborate and guide their care • Document electronically using keyboard and multiple computer screens

🎯 Requirements

• Candidates must reside in Arizona • 3+ years’ experience as a Registered Nurse • Active and unrestricted RN licensure in the state of residence • Compact Licensure RN License required • RNs residing in compact states must have an active multistate license through the Nurse Licensure Compact (NLC) • RNs residing in non-compact states must hold an individual state-specific RN license for each supported state • 1+ years nursing management experience with patients with Diabetes, Cardiac Disease, ICU, CCU, Home Health, or Pulmonary Disease • 1+ years’ experience documenting electronically using keyboard and multiple computer screens • Free-form text and good typing skills • Microsoft Office applications, especially Excel • Diploma Nurse or Associate’s Degree required • Bachelor’s degree preferred • Hard-wired internet required • Home Health experience preferred • Case Management experience preferred • Lexicomp experience a plus • Motivational Interviewing experience a plus • Certified Diabetes Care and Education Specialist (CDCES) preferred • Bilingual Spanish preferred
