📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Fulfill the requirements of the specified clinician role with Signify Health • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and intuitive clinical workflow • Anticipated weekly hours: 12
