📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits involving one-on-one health conversations • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Avoid prescribing medicine, ordering lab tests, or altering current treatment regimens • Build trusted relationships and help improve health outcomes • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Perform duties in health plan members' private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Must be flexible to travel locally and within licensed states, as needed • Must be able to conduct virtual visits as needed • Must fulfill the requirements of the specified role • MD or DO qualification explicitly indicated by the posting
