📋 Description

• Provide in-home health evaluations in members’ residences • Engage one-on-one with people and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Travel locally and within licensed states, up to 100% of the time • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Visit people nationwide and connect them to appropriate care • May require overnight hotel stays, car rentals, and/or flights

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within California and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Willing and able to travel locally and within licensed states up to 100% of the time • Multiple state licenses required as dictated by business needs • MD or DO qualification
