📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states, potentially up to 100% of the time • Build trusted relationships and help connect people to appropriate care • Make referrals to health plans and communicate health and quality-of-life information to primary care physicians as appropriate

🎯 Requirements

• Full-time Physician role for MDs and DOs • Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states, including overnight hotel stays, car rentals and/or flights as needed • Anticipated weekly hours: 40
