📋 Description

• Provide in-home health evaluations in members’ homes • Engage one-on-one with individuals and help bridge gaps in care • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel locally and within licensed states • Build trusted relationships and connect people to appropriate care • Visit people nationwide across a defined coverage area

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Flexibility to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to accommodate overnight hotel stays, car rentals and/or flights as required • Ability to conduct virtual visits as needed
