📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people one-on-one and help bridge gaps in care • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic and preventive tests when ordered, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services and Diagnostic & Preventive Services products • Travel locally and within licensed states, including possible overnight hotel stays, car rentals, and flights • Build trusted relationships and connect people to appropriate care

🎯 Requirements

• MD or DO physician qualification • Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to perform duties safely in homes that may contain household pets, secondhand smoke, airborne allergens or irritants • Ability to conduct in-home health evaluations, including medication and medical history review, physical evaluation, and diagnostic tests • Ability to use an iPad and clinical workflow • Ability to work 40 hours per week
