📋 Description

• Provide In-Home Health Evaluations in members’ homes • Travel across a defined coverage area, primarily up to 100% of the time • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute one-on-one visits • Review medications and medical history • Perform physical evaluations • Perform ordered diagnostic tests such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services • Visit people nationwide and connect them to appropriate care • Refer members to their health plan and encourage loved-one participation when appropriate

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to travel locally and within licensed states • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Ability to conduct virtual visits as needed
