📋 Description

• Provide in-home health evaluations in members’ private residences • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Engage members and build trusted relationships to improve health outcomes • Help connect members to appropriate care • Provide virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in members’ homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations
