📋 Description

• Conduct patient outreach through telephonic communication • Assist patients who cannot engage with the care model or are working with another provider • Help patients identify a suitable setting for primary care needs • Document patient interactions promptly in electronic medical records while adhering to HIPAA standards • Facilitate communication about complex topics among patients, families, caregivers, medical providers, and community organizations • Collaborate with patients and support persons to define goals, address impediments, and clarify expectations • Cultivate relationships with patients and caregivers independently • Partner with Community Health Workers, Social Workers, and clinicians to support complex patients • Educate patients about Oak Street’s comprehensive wrap-around services • Achieve a minimum target of 20 resolutions per cycle

🎯 Requirements

• Proven record of success in engagement, sales, and/or marketing within a healthcare-related context • Proficiency in verbal and written communication • Advanced technological acumen • Proficiency in Excel and other EHR/EMR platforms • Commitment to serving the community and establishing meaningful professional relationships • Ability to function autonomously in a demanding, fast-paced environment • Comprehensive understanding of Medicare and Medicare Advantage programs • Authorization to work in the United States • Spanish fluency in written and oral communication is considered advantageous • Ability to exemplify Oak Street Health’s core values
