📋 Description

• Provide medication therapy management services directly to patients • Conduct one-on-one phone/video-based consultations with commercial and Medicare Part D plan members • Assist with intake/enrollment processes as needed • Identify, resolve, and prevent medication-related problems • Assess medications for indication, effectiveness, safety, and adherence issues • Incorporate available lab data into medication assessments • Prepare medication summaries and personal care plans with recommendations for patients to share with medical providers • Follow up with patients to monitor drug therapy and ensure positive outcomes • Collaborate in a multidisciplinary practice with team case managers

🎯 Requirements

• Arizona Pharmacist License in good standing • Must reside in the State of Arizona • Excellent verbal and written communication skills • Well organized with the ability to effectively prioritize work • Customer focused • Ownership and results driven • Attention to detail • Proficient use Microsoft Office applications, e.g. Outlook, Excel, Word • Ability to effectively work within a team • Flexible in adapting to change • Ability to effectively plan, prioritize, organize time and workload • Skilled in identifying and solving issues and recommending process improvements • Ability to work successfully in a deadline-oriented environment • Pharm D Degree
