📋 Description

• Deliver holistic, evidence-based remote patient care through a virtual technology platform • Assess, diagnose, treat, and manage health problems for patients aged 18 months and above • Provide health counseling and disposition planning • Educate and counsel patients on pregnancy prevention, STI prevention, safer sex practices, contraceptive care, and medication management • Evaluate primary care, acute, chronically ill, and transitional care patients • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in Epic/EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues and the broader patient-centered medical home • Work within a care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

🎯 Requirements

• Current, unrestricted license in AL, AR, CO, IA, ID, IN, LA, MI, MS, NE, NV, OK, TN, or TX • Willingness and ability to obtain additional licensure if hired • Minimum of two years of medically relevant experience or equivalent • Completion of a Master’s Degree-level Family Nurse Practitioner program • Current National Board Certification • State of employment license to practice as an Advanced Practice Nurse • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multitask • Initiative, problem-solving ability, adaptability, and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Ability to collaborate with professional colleagues to provide quality care • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Minimum 9 hours of availability per week, including every third weekend • Ability to work evening shifts (2:30 p.m.–11 p.m.) and overnight shifts (9 p.m.–6:30 a.m.) • Broadband connectivity, quiet setting with neutral background, and ability to uphold patient privacy for remote care delivery • Ability to interact remotely with customers, use computer and phone, speak intelligibly, hear patient responses, and read computer screens and electronic devices
