📋 Description

• Deliver patient care services through a remote technology platform • Independently provide holistic, evidence-based care, including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning • Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices • Evaluate primary care, acute, chronically ill, and transitional care patients aged 18 months and above • Provide healthcare education, counseling, contraceptive care counseling, and medication management • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in an EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues and the patient-centered medical home • Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

🎯 Requirements

• Current, unrestricted license in GA, NC, SC, VA, NY, NJ, WV, CT, WI, or PA • Willingness and ability to obtain additional licensure if hired • Minimum of two years of medically relevant experience or equivalent • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multitask • Initiative, problem-solving ability, adaptability, and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues to provide quality care • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Completion of a Master’s Degree-level Family Nurse Practitioner program • Current National Board Certification • State of employment license to practice in the Advanced Practice Nurse role • Broadband connectivity, a quiet setting with a neutral background, and ability to uphold patient privacy for remote care delivery • Ability to operate computers and phones, communicate intelligibly, hear patient responses, and view/read computer screens and electronic devices
