📋 Description

• Conduct approximately 30-minute online Video Visits with health plan members, including medication and medical history reviews • Educate members about their health and answer questions • Help close gaps in care for Medicare Advantage, Medicaid, and certain commercial plan members • Provide 40 hours of availability weekly and consistently provide 320+ capacity units per month • Maintain a strong visit completion rate, pacing 220+ completed visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Obtain training and credentialing for health plan clients and lines of business in primary coverage states • Participate consistently in manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Conduct telehealth services from a private space with adequate broadband connectivity and enabled video/audio • Uphold health plan member privacy requirements • Operate computers, phones, iPads, and other electronic devices while interacting remotely with members

🎯 Requirements

• Full-time availability • Board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • Ability to obtain and maintain multi-state licenses in the coverage area, as required • FNP or AGNP certification preferred for nurse practitioners • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Ability to work 40 hours per week • Ability to provide 320+ capacity units per month and pace 220+ completed visits per month • Compliance with training, affiliation, credentialing, and Video Visit requirements • Broadband connectivity with video and audio enabled • Quiet setting with a neutral background for conducting visits • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology
