📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review members’ medications and medical histories • Educate members about their health and answer questions • Help close gaps in care and connect members to appropriate care • Provide 40 hours of availability weekly • Maintain 320+ capacity units per month • Consistently complete and pace 220+ visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Complete training and credentialing for health plan clients and lines of business in primary coverage states • Participate in regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Conduct telehealth services only from a private, quiet setting with adequate broadband connectivity • Operate computers, phones, iPads, and other electronic devices during visits • Uphold health plan member privacy according to CVS Health guidelines • Practice autonomously and work without direct supervision

🎯 Requirements

• Board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • Ability to obtain and maintain multi-state licenses in the coverage area as required • FNP or AGNP certification preferred for nurse practitioners • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar format preferred • Trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Availability for 40 hours weekly • Ability to provide 320+ capacity units per month and 220+ completed visits per month • Broadband connectivity with video and audio enabled • Private, quiet setting with a neutral background for conducting visits • Ability to protect health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Ability to sit, write, operate computers and phones, speak intelligibly, and hear member responses • Ability to view and read computer screens, iPads, and other electronic devices
