📋 Description

• Complete comprehensive health assessments • Manage complex patients with a focus on reducing hospitalizations and improving health outcomes • Coordinate and facilitate quality, consistent, cost-effective care • Support complex kidney patients in navigating the healthcare system • Deliver holistic, integrated care • Work autonomously and collaboratively with the healthcare team • Manage patients remotely across their kidney health journey, including early kidney disease, dialysis transition, transplant care, and life-sustaining dialysis • Document and code care from a Medicare Advantage risk perspective and conduct health risk assessments for high-risk or chronically ill patients • Provide longitudinal management of chronic illnesses such as CHF, DM, cardiovascular disease, and hypertension

🎯 Requirements

• Minimum of two (2) years' experience as a Nurse Practitioner (NP) • Current unrestricted NP license to practice in the state • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification • Intermediate computer skills • Proficiency in MS Word, Excel, and Outlook • Home office and high-speed internet • Willingness to obtain up to 6 additional state licensures for remote patient management • Passion for delivering care to patients with complex, chronic illnesses • Preferred: experience working in Value Based Care or Population Health • Preferred: documenting and coding from a Medicare Advantage risk perspective • Preferred: conducting Health Risk Assessments on high-risk or chronically ill patients • Preferred: 5+ years’ experience as a Nurse Practitioner practicing in adult/gerontology populations • Spanish-speaking preferred in some markets • Experience with longitudinal management of chronic illness, including CHF, DM, cardiovascular disease, and hypertension • Must complete pre-employment substance abuse testing and background verification checks
