📋 Description

• Treat patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups • Participate in continuous learning during weekly Clinical Rounds and through other modalities • Collaborate with virtual and in-office teammates via daily huddles • Apply specific clinical training to provide exceptional virtual patient care • Provide virtual clinical care in concert with patients’ primary care providers

🎯 Requirements

• Completed an accredited FNP program with a national certification • Currently licensed in NY, with possibility to obtain additional states • Practiced as an Advanced Practitioner for at least 2 years in the past 5 years in outpatient primary care seeing patients of all ages (0+), or in urgent care or emergency medicine seeing patients of all ages (0+) • Ability to work afternoons and evenings; current shifts are 7am-6pm EST or 11am-10pm EST • Excellent clinical and communication skills • Ability to follow established clinical protocols and workflows • Availability for the alternating Week A/Week B schedule, including rotating weekends • Ability to support 2 of 4 company-observed holidays • Passion for human-centered primary care • Ability to communicate with and provide care to individuals of all backgrounds • Ability to effectively use technology to deliver high-quality care • Clinical proficiency in evidence-based primary care • Desire to be an integral part of a team dedicated to changing healthcare delivery • Openness to feedback and reflection • Ability to navigate uncertain situations with patients and colleagues • Readiness to adapt personal and interpersonal behavior to meet patient needs • Must be authorized to work in the U.S.
