📋 Description

• Perform telephone triage using established protocols and sound nursing judgment • Assess symptoms, prioritize patient needs, and determine appropriate care or follow-up • Provide patient education, guidance, and reassurance • Document all patient encounters in Epic EMR in real time • Escalate urgent or emergent situations according to clinic and organizational policies • Communicate with on-call providers, clinic teams, and patients to ensure continuity of care • Contribute to safety, quality, and patient satisfaction within the triage team • Utilize the nursing process to assess, diagnose, plan, implement, and evaluate individualized care plans • Monitor patient status and modify or escalate care as necessary • Advocate for patients and uphold autonomy, informed consent, and self-determination • Manage relationships, mentor colleagues, and delegate care appropriately • Prioritize and organize time to optimize patient outcomes • Apply current evidence and standards to daily practice • Contribute to research, quality improvement, and evidence-based practice • Communicate effectively across practice formats and collaborate to produce positive outcomes • Precept nursing students, residents, and healthcare team members • Perform other duties as required or assigned

🎯 Requirements

• Active, unrestricted RN license • Multi-state compact eligibility/license required • Minimum 2–3 years of clinical experience with a strong background in triage, ER, Med-Surg, and/or Primary Care • Proficiency in Epic EMR and remote communication systems • Excellent assessment, critical thinking, and decision-making skills • Strong communication, documentation, and time management abilities • Self-motivated and comfortable working independently in a remote environment • Graduate from an accredited Nursing Program required • Basic Life Support (BLS) Certificate required upon hire • Reliable internet and a secure HIPAA-compliant workspace free from distractions • Licensed Registered Nurse with NH eligibility • Associate degree required; BSN preferred • Previous experience in remote triage, nurse advice lines, or outpatient/clinic settings preferred
