📋 Description

• Serve as the first point of contact for patients through onboarding, scheduling, and relationship-focused outreach • Facilitate CHE completion in nephrology offices, clinics, or via telehealth • Coordinate with Nurse Practitioners and clinics to prepare for in-person appointments • Make outbound calls to patients, caregivers, and clinics for CHE scheduling, reminders, and rescheduling • Schedule telehealth CHEs for patients unable to attend in person • Welcome new patients and collect concerns from existing patients • Escalate concerns to RNs or Nurse Practitioners as appropriate • Maintain accurate documentation and communicate patient updates to the care team • Build trust, influence patient participation, and overcome engagement barriers • Organize outreach by urgency and importance to maximize patient impact • Function across both ES and CC capacities

🎯 Requirements

• High School Diploma or GED required • Associate’s or Bachelor’s Degree preferred • Prior IKC experience required (CC/ES) • 2+ years professional work experience in a customer service, patient-facing, or telephonic outreach role preferred • Prior experience in a healthcare setting preferred • Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook • Strong verbal and written communication skills • Active listening and motivational techniques • Highly organized, with excellent time management and follow-through skills • Ability to work autonomously and collaboratively with care team members • Must reside in Pacific or Mountain time zones • Quiet, private home workspace with high-speed internet required
