• Perform clinical chart reviews to ensure documentation accuracy using established clinical criteria • Evaluate how provider documentation, laboratory results, diagnostic information, and treatment plans translate into coded data • Analyze and interpret clinical data to identify documentation gaps, inconsistencies, and improvement opportunities • Query providers using a concurrent query process compliant with ACDIS/AHIMA guidelines • Communicate with providers and healthcare team members to clarify information and obtain additional documentation • Work with clinicians to improve documentation quality and completeness through queries and provider education • Address disease processes, severity, complexity, and acuity for accurate medical coding • Collaborate with team members to promote accurate and consistent clinical review results • Meet accuracy and productivity metrics • Communicate documentation issues and trends to DaVita leaders • Participate in scheduled department meetings • Conduct individual and large-group educational sessions for clinicians, medical coders, and other applicable staff • Partner with data analytics, coding, compliance, and education departments on onboarding and ongoing targeted education • Educate IKC APPs and partner providers on Medicare risk-adjustment documentation requirements • Support the clinical documentation integrity leadership team in executing strategic goals • Travel 2–4 times per year as needed to present in-person training
• Associate's degree in nursing required • Current active clinical license (RN) required • CCDS-O or CDIP required or must be obtained within six months of hire • CRC required or must be obtained within one year of hire • 5+ years of clinical experience as a Registered Nurse preferred • 3+ years of clinical documentation improvement, coding experience, or equivalent required • 1+ year of experience working with advanced practice providers and other clinical and non-clinical staff required • Solid clinical background in evidence-based medicine, including chronic disease management and prevention • Experience in ICD-10 coding, documentation requirements, and risk adjustment • Knowledge of compliant query writing/process • Ability to perform comprehensive chart reviews of 20–30 medical records daily • Technical experience using EMR systems • Proficiency in Excel, Outlook, Teams, PowerPoint, and Word • Ability to travel for on-site meetings 2–4 times per year • Expert-level knowledge of Medicare risk adjustment, documentation, and coding requirements • 2+ years of experience providing risk adjustment education or management • 4+ years of experience developing educational content for clinical professionals • Associate's degree in nursing required • Comfortable presenting via Teams or in person • Strong technical skills, including Excel, Word, PowerPoint, and Outlook • Coding certification from AAPC or AHIMA, or additional coursework/experience in coding and risk adjustment • Expert experience working with Medicare Advantage programs and CMS Risk Adjustment model • Previous experience in clinical documentation review or a clinical quality program • Experience developing and delivering clinical education and training • Excellent analytical and critical thinking skills • Excellent verbal and written communication and interpersonal skills • Ability to work independently and as part of a team in a fast-paced environment • Motivated self-starter and creative problem-solver
