📋 Description

• Function autonomously and collaborate with all members of the healthcare team • Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients • Minimize fragmentation of the healthcare delivery system • Coordinate care between patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager • Provide telephonic, field-based, or combined telephonic and field case management • Conduct patient outreach nationally • Help prevent complications, reduce avoidable hospitalizations, and improve overall health for patients with kidney disease

🎯 Requirements

• A.D.N degree from accredited school of nursing required • Current Registered Nurse (RN) license in the state of practice required • Compact License preferred • MST or PST preferred • Willingness to adjust work hours as needed to accommodate patient outreach • Minimum of 1 year of IKC RN experience required • Ability to work a Monday-Friday schedule • Ability to work autonomously • Continuing education credits maintained as required by state of practice required • Minimum of three (3) years’ experience in clinical nursing required • Minimum of one (1) years’ experience in renal nursing or case management preferred • Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes • Demonstrated experience and effectiveness in change agent role • Demonstrated knowledge and understanding of CQI techniques • Current CPR certification required • Ability to adapt to new/electronic processes and increased productivity expectations • Basic computer skills and proficiency in MS Word and Outlook required • Functional proficiency with DaVita specific clinical software programs, including Capella, required within 90 days of employment • Home office with internet connectivity at minimum of 1MB upload and 1MB download speed required
