📋 Description

• Serve as the team's care traffic controller, tracking clinical needs to resolution • Locate in-network behavioral health services and schedule member appointments • Handle telephonic outreach and support with members and provider offices • Build and maintain relationships with community behavioral health partners • Confirm in-network availability and engage services on members' behalf • Provide ongoing administrative and outreach support during transitions from hospital to community care • Prepare for and contribute to weekly Interdisciplinary Team Meetings • Flag high-risk members, resource gaps, and next steps • Identify process problems, propose fixes, and partner on implementation

🎯 Requirements

• Associate's degree and 3+ years of work experience in a healthcare setting • Comfort multitasking on the phone — listening, talking, and typing simultaneously • Ability to independently manage a fully remote schedule, Monday–Friday, 8:30am–5:00pm CST, MST, or PST • Comfort learning new technology, including electronic health records and Google Workspace tools • Team-player mentality and confidence advocating for members with internal and external stakeholders • Prior behavioral health, Medicare or Medicaid, or complex and underserved patient experience is desired • Bilingual (English/Spanish) is desired
