📋 Description

• Deliver virtual telehealth acute care to members with emergent or critical illness • Diagnose complex medical conditions • Order and interpret diagnostic tests • Establish care plans and prescribe appropriate medications • Assess quality-of-care interventions, social needs, home health needs, and DME needs • Work with patients, families, caregivers, PCPs, specialists, pharmacy, clinical nursing, social work, and other Devoted team members to coordinate care • Use Devoted’s electronic health information system and provide feedback to improve the interface • Participate in weekend on-call support for clinical nurses in certain geographies • Support new clinical and organizational programs and initiatives as the role evolves • Integrate innovative virtual care practices

🎯 Requirements

• At least three years working as an APP, preferably in acute settings, urgent care, emergency medicine, and/or primary care or internal medicine with heavy acute management • Must be willing to work four 10-hour shifts per week, including one weekend shift per month • Working hours from 11am-9pm EST • Nurse Practitioners: active and in-good-standing RN and APRN licenses • Physician Assistants: active and in-good-standing Physician Assistant license • Required role licensure and certification in good standing • Ability to get licensed in requested states within 90 days of hire date • Willingness to obtain licenses in additional states as needed • Experience in primary care, internal medicine, urgent care, emergency room, and/or geriatrics • Experience performing visits over telehealth video platforms • Experience managing acute/chronic disease exacerbations, including CHF exacerbations, diabetic emergencies, COPD exacerbations, and hypertensive emergencies • Adaptability, eagerness to learn, and comfort working in a fast-paced, dynamic setting • Flexibility in scope of practice and willingness to embrace new challenges • Bilingual Spanish/English is a plus
