📋 Description

• Complete credentialing and privileging requirements for inpatient core privileges, as appropriate for role • Attend didactic lectures and educational opportunities • Provide direct patient care under clinical preceptor guidance • Collaborate and consult with physicians and other healthcare professionals • Manage an assigned patient caseload • Develop communication and presentation skills with the interdisciplinary team • Build knowledge of common disease processes • Identify professional development needs and create learning opportunities with preceptors and mentors • Conduct comprehensive patient health assessments, histories, and advanced physical examinations • Order and interpret laboratory and diagnostic tests • Order or prescribe medications, therapies, interventions, and procedures within clinical privileges and PA scope of practice • Document care and procedures in the medical record • Develop and direct individualized, comprehensive treatment plans • Coordinate discharge planning and after-visit care • Consult with specialties and disciplines to meet patient healthcare needs • Participate in committees, task forces, and professional organizations • Advocate for patients, families, nursing staff, and healthcare community members • Assist with clinical policies and procedures • Identify, evaluate, and solve clinical system and process issues with leadership • Develop, implement, and evaluate quality improvement strategic plans using evidence-based guidelines • Collaborate with physicians and nurses to lead changes in clinical practice • Teach and counsel patients and families on preventive care, medical conditions, care plans, outcomes, treatments, and medications • Provide formal and informal education to internal and external groups • Review and apply evidence-based research and guidelines to clinical practice • Participate in service-line quality initiatives and outcome metric development • Perform other APP duties as assigned • Complete a 12-month multidisciplinary Primary Care fellowship with clinical rotations, didactic curriculum, and supervised hands-on care

🎯 Requirements

• APRN-CNP: Graduation from an accredited Nurse Practitioner program • APRN-CNP: Meets all required qualifications of APRN-CNP job description • Master's degree from an accredited school/college of nursing • Registered Professional Nurse (IL) license • Advanced Practice Registered Nurse (IL) license • Adult-Gerontology Nurse Practitioner certification • Basic Life Support for Healthcare Providers certification from the American Heart Association • PA: Graduation from an accredited PA program • PA: Meets all required qualifications of PA job description • PA (IL) license • Current or past NM employee preferred • Interest in Primary Care, including hypertension, weight management, and geriatrics preferred • Completion of credentialing and privileging requirements for inpatient core privileges, as appropriate for role • Must complete a background check if offered the position
