📋 Description

• Deliver virtual telehealth acute care to members with emergent or critical illness • Evaluate and diagnose acute illnesses and complex medical conditions • Order and interpret diagnostic tests • Establish care plans, prescribe appropriate medications, and assess quality-of-care interventions • Assess social, home health, and durable medical equipment needs • Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers • Use the electronic health information system for visits and provide feedback to improve the interface • Participate in weekend on-call coverage in certain geographies to support clinical nurses • Support new clinical or organizational programs and initiatives as the role evolves • Integrate innovative virtual care practices

🎯 Requirements

• At least three years of experience working as an Advanced Practice Provider • Experience preferably in acute settings, urgent care, emergency medicine, primary care, or internal medicine with heavy acute management • Willingness to work three 12-hour shifts per week, including Saturday and Sunday, from 8am–8:30pm EST • Nurse Practitioners must hold active, in-good-standing RN and APRN licenses • Physician Assistants must hold an active, in-good-standing Physician Assistant license • Required role licensure and certification must be in good standing • Ability to obtain licenses in requested states within 90 days of hire and obtain additional state licenses as needed • Experience in primary care, internal medicine, urgent care, emergency medicine, and/or geriatrics • Experience performing visits over telehealth video platforms • Experience managing acute or chronic disease exacerbations, including CHF, diabetic, COPD, and hypertensive emergencies • Bilingual Spanish/English is a plus • Strong desire to continue practicing acute care • Adaptability, eagerness to learn, flexibility in scope of practice, and willingness to embrace new challenges
