📋 Description
• Conduct scheduled telehealth visits for residents in skilled nursing and assisted living facilities • Manage chronic medical conditions such as hypertension, diabetes, and COPD • Respond to urgent clinical requests via telehealth • Maintain accurate clinical documentation in EMR
🎯 Requirements
• Nurse Practitioner (NP) or Physician Assistant (PA) certification • Experience in telehealth, chronic disease management, or related fields • Ability to collaborate with interdisciplinary teams • Strong clinical assessment skills • Excellent communication skills
