📋 Description
• Provide patient care across the entire continuum of care • Triage Nurse making a meaningful impact caring for patients wherever they call home • Partner with Elara Caring in delivering the right care, at the right time, in the right place • Report to the Hospice Triage Supervisor
🎯 Requirements
• Graduate of an accredited School of Nursing • Current State License as a Registered Nurse or Licensed Practical Nurse required • 3 years of experience as a registered nurse in an appropriate clinical care setting and 1 year of supervisory experience • Experience in hospice or home health environment is preferred
