📋 Description

• Evaluate patients’ mobility, strength, safety, and functional needs • Develop and implement individualized treatment plans • Provide skilled physical therapy services in patients’ homes • Help patients improve mobility and safely perform daily activities • Educate patients and caregivers on exercises, equipment, and safety techniques • Monitor patient progress and adjust treatment plans as needed • Collaborate with physicians, caregivers, nurses, therapy professionals, and other members of the care team • Communicate changes in patient condition to the appropriate clinical team members • Complete accurate and timely clinical documentation • Supervise therapy assistants and support staff, as appropriate • Support patients in reaching their highest possible level of independence

🎯 Requirements

• Graduate of an American Physical Therapy Association-approved physical therapy program • Current and unrestricted Connecticut Physical Therapist license • Minimum of one year of experience in a healthcare facility • Strong communication, organization, and patient-care skills • Ability to work independently in a home-based environment • Willingness to travel throughout the assigned Norwalk and Fairfield territory • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
