📋 Description

• Provide primary and acute care through telephone and tele-video modalities • Develop and implement clinical plans of care for adult patients with chronic and complex conditions • Gather patient histories, physical examinations, and diagnostics as needed • Develop and implement treatment plans based on patient goals and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families about medications, side effects, illness progression, diet, nutrition, adherence, and crisis prevention • Coordinate care with company physicians, other physicians, medical providers, patients, families, and clinical team members • Maintain patient medical records and documentation in accordance with regulations and company policy • Participate in continuing education • Prescribe medication as permitted by state authority • Collaborate to develop complex care plans aligned with patient health status, goals, and values • Provide clinical and non-clinical support to patients receiving home and community-based services

🎯 Requirements

• MS in Nursing • Active, national NP certification • Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Virginia • Valid, current, active RN Compact license • 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) • Experienced Nurse Practitioner candidates • DEA registration or eligibility preferred • Active Medicaid number in Virginia highly preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Must reside within a reasonable commuting distance of the posting location(s), unless an accommodation is granted • Ability to work Monday–Friday, 8:00 am–5:00 pm EST or CST, with rotating on-call
