📋 Description

• Conduct clinical and health-risk assessments for patients with chronic and complex conditions • Develop, manage, and update individualized care plans • Monitor patient progress and RPM data and identify concerns requiring intervention • Provide patient education, coaching, and support related to chronic disease management and medications • Coordinate care with physicians, pharmacists, nurses, social workers, and other providers • Escalate clinical concerns and facilitate appropriate follow-up when needed • Maintain timely, accurate, and compliant EHR documentation • Support CCM/RPM quality, compliance, and patient engagement initiatives

🎯 Requirements

• Active, unrestricted California PA-C or NP license required • Current national certification (NCCPA, AANP, or ANCC, as applicable) • 2+ years of clinical experience in primary care, internal medicine, geriatrics, chronic disease management, or a related setting • Strong clinical judgment, communication, and patient-care skills • EHR proficiency • Ability to work independently in a remote environment • Reliable internet • Private, HIPAA-compliant workspace • Preferred: Experience with CCM, RPM, telehealth, population health, or value-based care • Preferred: Experience managing patients with multiple chronic conditions • Preferred: Familiarity with digital health and remote monitoring platforms
