📋 Description

• Work with the Home Health team to deliver individualized care to patients in their homes • Perform comprehensive patient assessments, including OASIS assessments at required intervals • Develop, initiate, and revise individualized plans of care as patient needs evolve • Deliver skilled nursing services and treatments outlined in the plan of care • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies • Collaborate with the interdisciplinary team to ensure seamless, holistic care • Conduct home health visits in Labelle, Lehigh Acres, Fort Myers, and surrounding areas

🎯 Requirements

• Graduate of an approved nursing program • Active Florida RN license • Minimum of 1 year of direct patient care experience • Prior home health RN experience strongly preferred • Strong assessment, critical thinking, and patient education skills • Willingness to participate in weekend and holiday on-call rotations • BSN or MSN degree preferred
