📋 Description

• Manage an assigned patient panel and address each patient’s specialized needs based on their conditions, healthcare needs, goals, and wishes • Manage overall care management of the patient panel using nursing and value-based care operations expertise • Establish trusting and empathetic relationships with patients and families, providing clinical and emotional support • Advocate and serve as a community liaison for patients while navigating the healthcare system • Perform assessments, identify patient and caregiver needs including social determinants of health, and create patient-centric care plans • Coordinate interdisciplinary care to achieve continuity, reduce fragmentation, manage kidney disease progression, utilization, and provider coordination • Monitor and evaluate care management plans and modify interventions as necessary • Follow evidence-based guidelines, workflow protocols, quality metrics, regulatory requirements, and payer requirements • Collaborate with physician partners, community providers, APPs, and other clinical disciplines on integrated care plans • Identify cost-effective measures supporting value-based care goals and resource utilization • Facilitate patient and caregiver education on treatment options • Support transitions of care and proactively address barriers • Participate in clinical huddles and patient care conferences • Engage in organizational process improvement and optimize care management workflows and patient engagement processes • Prepare reports and deliverables for stakeholders • Collect and deliver reports with program leaders on program success, patient outcomes, and patient/caregiver satisfaction • Perform other duties consistent with the role as assigned

🎯 Requirements

• Associate degree in nursing • Current RN License is required; Compact License preferred • Care management experience required • Certified Case Manager preferred • Intermediate skills with MS Office Suite of products including Outlook and Teams • Ability to work effectively in a primarily remote environment • Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps • Cable, Fiber, or DSL connections hardwired to the internet device are recommended • Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information • Must be able to work 8:30a–5p ET or CT • Support for patients in the Central and Eastern Time Zone
