Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct patient care in a telemedicine setting for obesity and comprehensive diabetes management • Manage insulin titration and patients using insulin pumps and continuous glucose monitors (CGMs) • Maintain an individual patient panel or collaborate with and supervise Endocrine APPs and Registered Dietitians • Review charts and conduct initial and return visits • Manage CKD/ESRD and CAD/CV conditions • Provide advanced treatment planning and medical management for complex endocrinology cases • Prescribe non-controlled obesity and diabetes medications • Ensure clinical documentation, protocols, and treatment pathways are followed • Support colleagues’ professional development, clinical growth, feedback, and continuing education • Participate in team-based care initiatives and quality improvement efforts • Attend departmental meetings

🎯 Requirements

• Specialized in Endocrinology and Obesity Medicine • Valid medical license to practice and prescribe legally in any US state • Eligibility to obtain additional state licenses as needed • American Board of Internal Medicine (ABIM) certified Endocrinologist • MD or DO degree • Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM) • 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology, post residency, OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of experience • Active and unrestricted Medical State License in any US State • Eligibility to obtain additional state licenses; IMLC preferred but not required • Currently opted-in with Medicare • Competency managing CKD, ESRD, and CAD/CV conditions • Expertise in CGMs, insulin pumps, and complex insulin regimens • Experience working with APPs, medical residents, or trainees in a clinical academic setting • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Open to working one of the following W2 roles: 32hrs/week, 36hrs/week or 40hrs/week • Experience providing synchronous patient care in a telehealth setting is preferred • Bilingual candidates with medical fluency in Spanish are desired, but not required
