Patients have access to a Board-certified weight loss physician and a Registered Dietitian to develop a tailored plan that encompasses behavioral health, nutrition, physical activity, and FDA-approved medications, if appropriate. Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs). • You have your own panel of patients, and the role could also involve collaborating with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes. This role could involve APP collaboration and supervision. • Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions. • Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols. • Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs. • Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned Endocrine-Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship. • Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations. • Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine. • Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care. • Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.

🎯 Requirements

• Board certification in Endocrinology by the American Board of Internal Medicine (ABIM) • Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience (minimum 3 years) in obesity medicine and pharmacotherapy (older generation oral and newer injectable medications). • Active and unrestricted Medical State License in either of the following states AL, TN, OK, MO, IN, SC or GA, and currently holding a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact) • Minimum of 5 years as an attending physician in endocrinology. • Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions. • Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens. • Strong interest in mentoring, teaching, and collaborating with Endocrine-Specialized APPs and Registered Dietitians specializing in diabetes. • Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting. • Excellent communication and interpersonal skills. • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity. • Committed to delivering patient-centered, culturally competent care. • Bilingual candidates with medical fluency in Spanish are highly preferred
